What to Expect on Your First Appointment
Each individual’s orthodontic situation is different and deserves personalized attention. Your initial visit is an important opportunity for us to get to know you and your unique smile goals. We believe that orthodontic problems involve more than just teeth, so we look at the entire facial structure when treating our patients.
On your first visit, Dr. Kozak or Dr. Durbin will perform a comprehensive orthodontic examination to determine the status of your orthodontic health. If he determines that orthodontic care is necessary, we will make a recommendation based on your specific situation, explaining the procedures involved, how long treatment should take, and the cost and payment options available to you. Our goal at Kozak Orthodontics is to educate our patients so they may make an informed decision that best suits their goals.

We base each individualized treatment plan on your health history, clinical examination, and diagnostic records. This enables us to make informed decisions in determining what type of appliance will provide the best results.

Kozak Orthodontics uses the latest in diagnostic technologies, treatment planning, and appliances. Dr. Kozak and Dr. Durbin see each patient, each visit. Please Bring These with you to Your First Visit
- Complete medical/dental history document
- Know your insurance benefits and eligibility
- X-rays and referral slip from your general dentist if you have them
